Making Connections
Making Connections is designed as a transitional program as students prepare to leave the school setting and enter adult day services.
Students of the Making Connections Program can work on academics as well as establish a strong base of life/independent living skills and pre-employment skill development.
At age 21, students graduate from the Making Connections program and have the option to continue their day program in Network 4 Change’s Healthy Independence Program (HIP).
Students between the ages of 13-21 are directly referred by their School Division student support services teams and administration.
